Absolutely, there were some very poor choices that were made, which led
to the Exxon Valdez disaster. A lack of assistance from the Coast Guard
and Hazelwood’s choice to not follow established the lanes were
critical. There were some overall policy changes
as well for the oil transportation industry over the years which led to
these types of problems.  The report identified a few of them. What
standards had changed? How did these changes come about??  There is some
interesting information under the sections
“History: Oil in Alaska” and “Preparedness: Alyeska’s oil spill
contingency plans.”

As for the Bhopal incident, is it clear that it was an issue of poor management?
