Clinical Experience Week 2
This week in clinical practice allowed me to immerse myself into patient care, deepening my understanding of the role of an advanced practice nurse. I interacted with a variety of patients, each presenting unique medical challenges, which enhanced my ability to adapt and apply theoretical knowledge in real-world scenarios. Additionally, this experience underscored the importance of communication and empathy, vital traits for effective patient care.

During my rounds, I encountered several challenges, particularly when it came to managing complex cases. One notable success was my ability to assist in implementing a care plan for a patient diagnosed with chronic obstructive pulmonary disease (COPD), which resulted in improved patient outcomes. This success reinforced my confidence in utilizing evidence-based practices in patient care, fostering a sense of accomplishment.

Assessment of the patient revealed significant signs and symptoms (S&S), including wheezing, shortness of breath, and decreased oxygen saturation levels. A thorough physical examination, including auscultation of lung sounds, confirmed these findings, highlighting the urgency of the situation. The health team and I developed a comprehensive plan of care aimed at stabilizing the patient, incorporating bronchodilators and corticosteroids, and considering oxygen therapy as needed. This process emphasized the importance of timely and accurate assessments in guiding treatment decisions.

In terms of differential diagnoses, I proposed three potential conditions that could explain the patient’s presentation: pneumonia, asthma exacerbation, and heart failure. Pneumonia was considered due to the patient’s cough and fever, while asthma exacerbation was noted due to the wheezing and history of asthma. Heart failure was included based on signs of fluid overload. These rationales grounded the clinical decision-making process in a comprehensive understanding of respiratory conditions.

To promote the patient’s health, I focused on educational interventions that addressed smoking cessation and the importance of adherence to prescribed medications. Health promotion was tailored to the patient’s lifestyle, encouraging them to participate in pulmonary rehabilitation to improve lung function. This intervention not only aimed to manage the current condition but also recognized the need for long-term health improvements, making it a crucial aspect of my care plan.

From this week’s clinical experience, I learned the significance of collaboration within a healthcare team and the impact it has on patient outcomes. As an advanced practice nurse, fostering a cohesive team environment is essential for delivering high-quality care. This experience reiterated the importance of ongoing education and staying current with best practices in clinical guidelines, allowing for more informed and effective patient interactions.

Supporting my plan of care with current peer-reviewed research, I reviewed articles emphasizing the implementation of evidence-based strategies for managing COPD. Research underscores the need for personalized care plans that consider individual patient circumstances while integrating contemporary clinical guidelines. This evidence lent credibility to my approach, ensuring that patient care is rooted in reliable and up-to-date knowledge.
