1. Clinical Manifestations and Subjective & Objective Data:
List the symptoms and signs your patient is presenting. These could include physical symptoms, behavioral changes, or anything else relevant to the patient’s condition. Subjective data involves the patient’s self-reported experiences, while objective data includes measurable observations by healthcare professionals.

2. Patient Data: PMH and Significant Findings:
Provide a summary of the patient’s medical history (PMH) and any relevant past medical conditions. Highlight findings that are directly related to the current health issue and may impact the patient’s care.

3. Nursing Diagnosis and Measurable Goal:
Choose an appropriate nursing diagnosis using NANDA wording. Make sure the diagnosis accurately reflects the patient’s condition. Define a measurable and reasonably attainable goal that can guide the patient’s care.

4. Laboratory and Test Data:
List any relevant laboratory tests, imaging, or diagnostic results that contribute to the understanding of the patient’s condition. These results can help guide treatment decisions and interventions.

5. Interventions based on Collaboration and Interdisciplinary Care:

If you believe the suggested improvements will enhance the patient’s outcomes, explain why. Conversely, if you’re skeptical about the effectiveness of certain collaborations, provide your reasoning.
