Introduction
Case management and care management are vital areas in the delivery of care to the public. A family represents people with common ancestry characteristics or relationships. For instance, a relationship between parents and children or spouses defines a family set up. There are various groups of families depending on the relationship including parent and child, spouses or relatives. One of the models of care for families is the family-centered approach (Thyrian et al., 2017). The approach focuses on the medical as well as psychological needs of patients plus the impact of the family on the health condition.
Family Level to Aggregate Level
The strategy of moving from the family level to the next level is understanding that dynamics change from one level to another. Healthcare professionals should carry out an in-depth assessment to determine the health needs of an aggregate level. Other methods include enhancing the coverage and provision of resources. Healthcare services provide care to patients using different approaches, such as care and case management. The two approaches provide custom care to different categories of patients.
