A 15–year–old male reports dull pain in both knees. Sometimes one or both knees click, and the patient describes a catching sensation under the patella. Musculoskeletal pain is a common issue for many patients, and assessing the causes of pain in this system can be challenging due to its interconnectedness. To properly diagnose and treat musculoskeletal pain, a thorough assessment process that includes patient history and physical exams is necessary.

In this case, a 15-year-old male reports dull pain in both knees, and sometimes experiences a clicking or catching sensation under the patella. To gather more information about the patient’s condition, additional history such as the duration of symptoms, any previous injuries or treatments, and any aggravating or relieving factors should be collected.

To differentiate the causes of knee pain, categories such as joint pain, muscle pain, and ligament or tendon pain can be used. Specific differential diagnoses for knee pain include patellofemoral pain syndrome, osteoarthritis, and ligament or meniscus tears.

During the physical examination, the patient’s range of motion, stability, and alignment of the knee joint should be assessed. Special maneuvers such as the McMurray test and the Apley test can also be performed to further evaluate the patient’s condition.

Five possible conditions that may be considered in a differential diagnosis for the patient include:
Patellofemoral pain syndrome
Osteoarthritis
Ligament or meniscus tears
Osgood-Schlatter disease
Chondromalacia patellae
In addition to the physical examination, diagnostic tests such as X-rays, MRI, or an ultrasound may be necessary to gather more information and make a diagnosis.
